Download the Z83 form

Download the official Z83 application form from dpsa.gov.za. Complete it in full — all sections must be filled in. An incomplete Z83 will disqualify your application.

Gather certified copies of your ID, qualifications, and any other certificates. Certifications must be done at a police station or commissioner of oaths. Do not submit originals.
